International Visitor Request Form
Please complete the form below if you would like to invite a foreign national to Juilliard. The O.I.A. will use the information to determine the appropriate visa type for the foreign national.

Once reviewed, the O.I.A. will provide you with a list of documents that must be submitted to our office prior to the visit, which may include a completed English Language Assessment. A copy of the rubric can be found here. If the O.I.A. determines that a J-1 visa is necessary, and the visitor is not a citizen or permanent resident of a country for which English is an official language, the rubric must be completed prior to the issuance of the DS-2019 (the form created by the O.I.A. and used by the visitor to apply for a J-1 visa).
