For students participating in Optional Practical Training (OPT), please keep in mind that you must report changes in employment, U.S. address, name, and visa status within 10 days of the change.

Complete this form to update the O.I.A. with this information. If you change your visa status or the spelling of your name in your passport, please email the O.I.A. a copy of your new paperwork so that your immigration and Juilliard records can be updated.
